<strong>Overview<br><br></strong>The Remote Patient Monitoring (RPM) RN plays a critical role in supporting the Care Management Program by collecting and analyzing biometric data to monitor patient health status. This role involves telephonic clinical assessments, patient education, and proactive care coordination to ensure timely interventions and optimal health outcomes.
