Duties

  • Conduct initial assessments of patient needs and determine appropriate care coordination interventions.

  • Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.

  • Organize and maintain comprehensive patient records, documenting all interactions and care plans.

  • Monitor patient progress and adjust care plans as necessary based on changing needs or feedback.

  • Coordinate referrals to specialists and community resources, ensuring patients have access to necessary services.

  • Participate in team meetings to discuss patient cases and collaborate on improving care delivery.

  • Evaluate the effectiveness of care coordination strategies and contribute to quality improvement initiatives.

What we need for this special role.

  • Bachelor's Degree in Healthcare or a related field.

  • Strong communication skills, both written and verbal, to effectively interact with patients and healthcare teams.

  • Experience in care coordination or a related healthcare position is preferred.

  • Familiarity with electronic health records (EHR) and other healthcare management software.

  • Knowledge of healthcare regulations and best practices in patient care coordination.

  • A minimum of 1-3 years of experience in a healthcare setting is preferred.
