Position Title: CareCoach (Virtual Medical Assistant)

Position Summary:

We’re looking for a compassionate and dedicated CareCoach to support our patient engagement team. In this role, you’ll guide patients in following their care plans, encourage healthy habits, and educate them on managing their conditions. You'll also assist patients by monitoring their vital signs while working with Electronic Medical Records (EMR) systems. You will be expected to meet a minimum number of engagements per day to ensure each patient is engaged with on a bi-weekly basis.

Key Responsibilities:

1. Patient Support and Empathy:

  • Provide empathetic and patient-centered care to individuals with chronic conditions.
  • Support and reinforce the care plan developed by healthcare providers.
  • Build strong relationships with patients through regular communication and follow-ups.

2. Healthy Lifestyle Coaching:

  • Educate patients on healthy lifestyle choices, including diet, exercise, and medication adherence.
  • Offer personalized coaching to help patients achieve their health goals.
  • Provide education on the patient’s specific conditions to promote better self-management.

3. Vital Monitoring Encouragement:

  • Encourage and guide patients in using Hbox provided vital monitoring tools such as blood pressure monitors, pulse oximeters, and glucose monitors.
  • Assist patients in understanding and tracking their vital signs to manage their health effectively.

4. Care Coordination:

  • Coordinate patient care across multiple providers and services to ensure comprehensive and continuous care.
  • Facilitate communication between patients, families, and healthcare providers to address patient needs and preferences.

5. EMR Proficiency:

  • Efficiently work with various EMR systems, including EPIC, eClinicalWorks, and Practice Fusion.
  • Document patient interactions and care plans accurately and promptly in the EMR systems.

6. Passion for Care Delivery and AI Technology:

  • Demonstrate a passion for delivering high-quality care and improving patient outcomes.
  • Show a keen interest in the integration of AI technology in healthcare and its potential to enhance care delivery.
  • Be open to meeting daily engagement quotas backed by successful data analytics.

Qualifications:

  • Certified Medical Assistant (CMA) certification is required.
  • Experience in remote patient monitoring, chronic care management and patient education.
  • Strong understanding of various chronic conditions and their management.
  • Proficiency in using EMR systems such as EPIC, eCW, Practice Fusion.
  • Excellent communication and interpersonal skills.
  • Ability to work independently in a remote setting.
  • Passion for patient care and curiosity about advancements in AI technology.
  • Bilingual proficiency in Arabic or Spanish is strongly preferred, but not required.

Pay: $18.00 - $20.00 per hour
